It is MANDATORY for all volunteers involved with BYSA to have a CORI/Kidsafe form on file with Mass Youth Soccer.

This only needs to be done ONCE - not annually or per season.

The easiest way to do this is online at the MYSA website:

Alternatively, you can download a paper form at the MYSA site and mail it in.
